To make your first visit with us as streamlined and efficient as possible, you are welcome to print out your new patient forms and fill them out at home. Please also bring your insurance card or any other applicable financial information. Patients who arrive more than 15 minutes later than their scheduled appointment time may be asked to reschedule, and we reserve the right to charge a cancellation fee for anyone who does not provide at least one business day’s notice before canceling their appointment.

It can be difficult to pay for dental treatments upfront, so to help patients receive the dental care they need when they need it, our office accepts third-party payment plans through CareCredit.
